============================================================================================================== ADMISSION GUIDELINES
Admission to San Francisco Institute of Science and Technology is a privilege. The institute therefore reserves the right to refuse admission to applicants or to require the withdrawal of trainees when it deems their presence among the trainee body is inimical to the objective of the institute or to the observance of the rules and regulations.
All entrants are required to accomplish admission application form and take SFISTAT (San Francisco Institute of Science and Technology Admission Test).
A. FOR INCOMING FIRST YEAR TRAINEES: 1. SFISTAT Result 2. Form 138-E (High School Report Card) 3. Certificate of Good Moral Character 4. Barangay Clearance 5. Birth Certificate from NSO (Xerox copy) 6. Medical Certificate (from Government Physician - Original) 7. 3 pcs. 2 x 2 I.D. Pictures 8. 1 pc. long size plastic brown envelope
B. For Transferee (Addt’l Requirements other than A) 1. Certification of Grades in lieu of Official Transcript of Records 2. Certificate of Honorable Dismissal/Transfer
C. FOR RETURNEE/SHIFTEES 1. Duly Signed Trainee Clearance 2. No failing grade in two or more competencies in his/her present course (shiftees) as Certified by the School Registrar. 3. Certification of Previous Performance issued by the SFIST Guidance Counselor. 4. Acceptance Letter of the Course Adviser/Department Head where he wants to shift.
Renewal of registration of returnee/shiftee trainees is dependent on his previous performance. Refusal of re-enrollment shall be applied for violation of the rules and regulation of the institute or scholastic/academic deficiency which means failure to pass two or more competencies of the total number of competencies taken in any given semester or school term.
D. OLD TRAINEES:
1. Duly Signed Trainee Clearance 2. Performance Appraisal for trainees with OJT/SIT during summer.
E. Foreign Trainees:
1. Original Transcript of Record 2. Personal Data 3. Affidavit of Support from Guardian/Parent 4. Alien Certificate of Registration (ACR) 5. Results of Test on English as a Foreign Language (TOEFL) for non-speakers of English 6. Study Permit from Concerned Agency 7. SFISTAT Result 8. Certification of Completion of a secondary curriculum where the applicant had his or her secondary education.
ENROLLMENT
A. PRE-ENROLLMENT PROCEDURE
Trainees shall be admitted for enrollment upon qualifying the SFISTAT to be given by the testing committee starting January to March of the calendar year before the enrollment period of the first semester of the current school year. Result shall be released thru mail in its corresponding addresses starting the month of May until such time that enrollment period starts.
B. REGISTRATION
Qualified applicants for admission should register within the period of registration. Registration includes filling out of the prescribed forms and paying the required fees.
Trainees enrolled during the preceding semester including summer session are re-enrolled upon presentation of clearance duly signed by concerned instructors and personnel.
Late registrants shall present an approved letter by the Vocational School Superintendent/Vocational Instruction Supervisor to the Office of the Registrar, requesting of his/her enrollment and shall be imposed to pay late registration fee.
REGISTRATION PROCEDURE
Step #1a: Secure Enrollment Procedure Form from the Registrar’s Office (Incoming First Year Trainees) upon presentation of the complete admission requirements. Step #1b: Submit for evaluation of unit of competencies taken and grades (previous semester—for old/returnee trainees) at the Registrar’s Office then secure Enrollment Procedure Form provided they complied with the admission requirements. Step #2: Proceed to the Clinic for Physical Examination. Step #3: Proceed to the Interview Committee for Interview. Step #4: Secure preliminary and white form from Registrar’s Office and submit the completed admission requirements. Step #5: Fill out the preliminary and white form and copy the units of competency to be enrolled. Step #6: Proceed to Registrar’s Office for verification of the filled out preliminary/white form. Step #7: Proceed to the Accounting Office for assessment of school fees. Step #8: Pay the corresponding amount to the Cashier’s Office and secure Official Receipt as proof of payment. Step #9: Proceed to the Registrar’s Office for Approval and stamping of “ENROLLED” in the White Form.
Note: Lost Class Cards will be paid upon request by the trainee concerned.
